PROPOSED BENEFITS OF STOCKHOLDERS AND HIS/HER SPOUSE

1. Free board and lodging up to suite room accommodation
2. Professional fee is 50% of the house case rate.
3. No operating and delivery room rate
4. Fifty percent (50%) discount on X-ray and laboratory procedures.
5. For pharmacy, CSR, O.R. materials the discount is computed as follows:

Cost of acquisition plus 10%

1. Fifty percent (50%) discount on professional fees for OPD consultation.
2. For dental services, free consultation, thirty percent (30%) on the following procedures: Restorative, Preventive, Periodontics, Endodontics, Surgery and Bleaching; and 10% discount on Prothodontics procedures.
3. For medical stockholders, after payment of right to practice: (a) hold clinic in the hospital for a maximum of 8 hours a week paying only the     maintenance, water and electric bills;(b) rotation in the House Cases.

Stockholders’ Natural Parents and Dependents 

1. Free board and lodging up to suite room accommodation.

4. Twenty five percent (25%) discount on X-ray and laboratory procedures.
5. Twenty five percent (25%) discount on cardio-pulmonary service and ECG.
6. For pharmacy, CSR, O.R. materials the charges shall be computed as: Cost of acquisition plus 10%
7. Fifty percent (50%) discount on professional fees for OPD consultation.

The above benefits shall be enjoined for a duration of forty-five (45) days per stockholders / dependents per year, non-cumulative, non-transferable, no carry over.

The above privileges shall commence only upon full payment of shares of stocks subscribed, certified and verify by the Corporate Secretary or his/her duly authorized staff.

Every stockholder is enjoined to submit the names of their legal dependents with the following documents: Marriage License (if married), Birth Certificates and other pertinent papers.

Definition of Dependents:  Legitimate single children less than  age 21 years;

Legally adopted children; Natural parents of stockholder
